Why did they pull back the mandible instead of just advancing his upper jaw
Imaging going through braces, bimax, and brutal recovery just to be a recessed birdcel

Nice Avi, can I fuck ithe is not meant to ascend
nothing can be done, bimax is not some miracle
yes fuck itNice Avi, can I fuck it
He apparently got a 10mm lefort (according to his reddit post).Generalized maxillary hypoplasia i.e. there’s little advancement to do without turning him into a flat nosed chimpanzee.
Conversely birdcels with normal nasomaxillary development will have enough nose to absorb large advancements and typically end up getting great results, given appropriate movements.

I saw, sorry if unclear - wasn’t responding directly to OP’s question, just thoughts on the case.He apparently got a 10mm lefort (according to his reddit post).
If he got lefort 2, wouldn't his nose project further than his forehead and look completely out of place? He already has a feminine nasofrontal angle.I saw, sorry if unclear - wasn’t responding directly to OP’s question, just thoughts on the case.
But yeah, checks out with how excessively full the paranasal area is and the nose flattening. Way too big of an advancement in terms of aesthetics and likely done per standard OSA protocol given it’s +10mm exactly.
I can’t say it’s a bad result given his shit base though. Could do a bigger movement altogether to preserve the mandible and opt for hardcore nasal reconstruction after, but 10mm is already pushing the limit on advancement anyway, unless he were to opt for DO. Smaller movement = more off the mandible and his already looks like shit from losing just 5mm. Perfect LF2 patient but few would’ve operated on a non-syndromic.
Can probably throw in a little rotation to improve the naso-frontal no?If he got lefort 2, wouldn't his nose project further than his forehead and look completely out of place? He already has a feminine nasofrontal angle.
Possibly, but it would be a marginal improvement to what he got with lefort 1. I don't think he would be able to get much advancement at his nasal bone since it wouldn't take much for him to be at a 180 degree nasofrontal angle. Can probably throw in a little rotation to improve the naso-frontal no?
Depends on what movement is appropriate and impossible to say w/o his scans. But due to the height of the maxilla (Nasion-to-ANS/U1 incisal tip, etc), with LF2 you should able to eek out a relatively large amount of advancement with a relatively small CCW rotation of the mobilized complex occuring at Nasion (this would decrease the Nasofrontal angle and increase nasal bone projection).Possibly, but it would be a marginal improvement to what he got with lefort 1. I think it's his frontal bone that's the issue tbh.
