For acne go in this order, in case the previous doesn't work:
1) remove all sugar from diet including fructose and lactose
2) topical solutions like tretinoin
3) oral accutane if nothing works.

Yeah, he'd get the best immediate results from a rhino, but from maximizing the end gains it's always wiser to go for osteotomies first since they change soft features too. If he's a poorcel then maybe just saving up for a rhino is for the best.
