He needs implants to zygos and jaw to make his face wider, ear pinning and some sort of jaw work for malocclusion.
His eyes are at least wide enough apart to allow for face widening without making him a Cyclops.

He doesn't have flaws that require extreme surgery. So it's not that bad. But he's still 2/10 as it is.
