Fat transfers have increased in popularity and for good reason, but you need to understand that MOST surgeons who offer facial fat grafts mainly do female faces, as a male, there should be specific points to target, and specific points to avoid.

AVOID:

- Fat transfers to the submalar / cheek area should be avoided, it goes without saying that this will kill your dimorphism if you had any to begin with, make you look bloated and fat like the poor guy below


Fat grafting side view

- Avoid fat transfers to the jaw, fat doesn't behave the same way filler does, and will not enhance your jawline, again, it will have the opposite effect and remove definition from arguably the most important place to have it

- Avoid the chin, most chin fat transfers (from what I've seen) have resulted in a round "ball" like chin, genio or death for the chin I'm afraid

DO:

Upper peri-orbital fat transfer

- Arguably the best use of fat grafts for correcting upper eye hollowing (superior sulcus fat loss), death tier falio btw, while also providing ideal hooding, great health indicator too, upper eye fat pads are present in young healthy people

Under eye fat transfer

- Can reduce under eye hollows and dark circles, provide an overall healthier looking face, while improving perceived depth of the eyes by fixing orbital vector, see below

"Recessed infras" are hardly a thing, truly recessed infras come from recessed maxilla's which most of you don't have, as shown in the image, orbital vector is mainly the product of fuller, healthier fat pads under the eyes, here's an example of orbital vector being changed with fat transfers

*Note: Under eye transfer can supposedly raise the lower lid 1-2mm however this is hardly definitive and I would not count on this happening

Brow ridge

- Though seemingly rare (I haven't seen many results online), the results I have seen have been good, Eppley wrote about this, adding slight volume to the brow to make it more defined can be a subtle yet nice looksmax

Outer brow (hardly spoken about but important to consider)

- High volume of fat in the outer brow is a trait I've in many attractive people and seems to be (in addition to good bones) one of the reasons they have such striking eyes and positively tilted brows, the best way to explain is with a picture, and the best example I can think of is Kit Butler

See how despite overall leanness, his upper eye and outer brow fat pads remain full and healthy? See how the increased volume at the most lateral part of the brow causes that distinctive tilt? I believe this could be a big looksmax if your face suits it

Lips

- Pretty low survival rate for fat cells as far as I'm aware, but still worth considering, even if you can get 10-20% more volume it a w in my book

Temples

- Another good place to get grafts if you have temporal hollowing, again a small touch that can at the very least fix a slight falio

A quick note on buccal fat removal / reduction

While on the topic of facial fat I just wanted to add a quick note on buccal fat. It is understood that bones are the primary focus of facial attractiveness, but unless you have death tier craniofacial development, and are between 10-12% bodyfat, yet still have that typical "oval" face shape, where the horizontal width of your face peaks at your cheeks (not at the bi-zygoma), then buccal fat removal is a very good surgery, and anything but a meme surgery as these single digit iq mouth breathers will have you believe.

"B-b-buttttt what about when I'm older and my fat pads start to" bla bla bla. This in my experience is fucking nonsense, most humans are born with too much buccal fat, and that buccal fat REMAINS as you age. All my great uncles, my grandfather, have all experienced fat loss in the upper and under eyes, the arms, legs and stomach but their cheeks remain full as ever. Just don't go overboard with it like that one actress with the fucked ipd.

**If you made it this far then you're probably serious about considering fat grafting, in which case please read up on the safety measures and risks associated with injecting fat into the face, mainly around cannula sizes, injection points, etc. Good luck and I wish you the best results <333

I agree with what you said about lots of fat into the lateral orbital rim, makes your upper third appear more robust imo.


What’s your opinion on getting aegyo sal via fat grafting, it’s a common surgery in Asia, my surgeon even recommended it.

I wanna try morph Aegyo Sal onto my face, but idfk how to morph that, such a hard thing to morph.

I've seen some good results. Just make sure you choose an experienced injector with a steady hand.

Well they are, but most people don't have bad enough recession to warrant implants, fat alone can make up the difference as shown in the example

Interesting, that might explain why infra implant results seem underwhelming to me.

@RealSurgerymax you often discourage getting any sort of filler, what’s your take on something like this?

For the past three or four years, it’s seemed to me that people have been trending away from fillers (much like buccal fat removal), but I’ve seen more than a few Ben Talei, Andrew Jacono-level doctors starting to say this doesn’t age the best either.

Fat ages much better than fillers.

Fat grafts are for soft tissue deficiency and a small amount is absolutely necessary in many people to buffer the skin from the orbicularis oculi so you dont get fine lines and wrinkles.

Coke is also getting popular again, especially among gen Z (not good but reality) and they need their under eyes filled with fat.

They do not replace implants. They do different things.
