its not as rare as lefort 2 and i know a handful of surgeons in france and italy that do provide it, though there is another strange form of a bsso cut that can increase ramus height but its usually if you dont need any advancement

Maybe look into an L shaped BSSO? But only if your ramus is the problem, and you have no functional issues, then it doesn't really make much sense to do it.
