My FWHR = ca. 1.6
Ideal FWHR = +1.8

My Midface Ratio = ca. 0,86
Ideal Midface Ratio = 1.0 - 1.1
Hey everyone!
Im 17 years old and I went to the orthodontist (not a normal one, a cosmetic surgery orientated one) today with the hope of getting MARPE to raise my FWHR. We did CT scans and 3D teeth scans and photos in all types of angles (all for free!) yet I could only look at the pictures and not take photos of them or I would have needed to pay money -so I will try to remember the photos I saw the best I can. I didnt want to wait till im 21 to get SARPE or have to pay a lot of money for surgeries that I could have prevented with easily getting MARPE. The doctor (female), said that MARPE is stupid in my case. My intermolar width is around 39mm and my bite is pretty good-normal. She said that it doesnt make any sense and even if I would have gotten MARPE I would have needed to surgically expand my lower jaw to correct the bite, since the upper jaw would have become way wider than the lower jaw -I hope you understand me JFL. So even if I would do MARPE + surgically expanding the lower jaw it would have costed me more than 7k or smth she said i.e. I dont have the fucking money so I suppose I wont get MARPE.
She also said that I have a class 2 malocclusion (I think, dont get me wrong my teeth align perfectly but the mandible is still recessed)

NOW here is the thing!
She knew that I wanted more bizygomatic and bigonial width so she proposed me mandibular advancement! Mandibular advancement without surgery -yeah I never heard about that JFL.

So I will try to explain the "advancement" that happens without getting surgery:

Which will keep my mandible in an advanced position.

I couldnt find any good picture but imagine it somewhat like this. The space I circled in red represents the new created space due to the mandible being in a constant forward position. In that new created space over time new bone will get build. After constantly wearing the retainers for at least 11 months, without moving the jaw backwards at any time to prevent the new bone structure of getting crushed, new bone will get created keeping the jaw in the position which the retainers gave me -without then using the retainers anymore.

The problem:
Obviously after getting rid of the retainers, the teeth of the upper jaw will exactly be above the teeth of the lower jaw and I wont have a proper bite.

To fix that, the teeth of the upper and lower jaw -i think, well at least one of the 2- will move backwards into the gingiva/gum and I will obtain a perfect bite again (not soly alone but with the help of another pair of retainers i suppose).

Another problem is that I have in the upper left jaw 1 wisdom teeth and in the lower jaw on each side 1 wisdom teeth -in total 3- she said that with the wisdom teeths being there, that there wont be a chance for the other teeth to move back into the gum since the wisdom teeth are an obstacle. So the following solution is wisdom teeth removal, from what I have heard wisdom teeth removal descends you? -I wanna hear thoughts on that pls- and yeah thats the whole thing to fix my recessed side profile and give me a slightly wider face. Im obviously not expecting my face to become a lot wider but at least a bit XD

Wisdom teeth removal = 750,60€ = 881,04 USD
Retainers = 550€ = 645,58 USD
All of the other procedures (cleaning, scans etc.) = 11039€ or 12.955 USD before discount -> 4935,10€ or 5.790,67 USD after discount -yeah I also dont know why they give you so much discount haha.

Total cost = 6235€ = 7.317 USD


I suppose after all that I still need braces to completely fix angle and position of the teeth. I also wanted to add, that I said to her "I want the jaw to be moved even a little bit more so that I have more forward projection, but she said that the Edge to edge bite position is the most advancement she can do and that if its to much advancement that it wont be possible. I didnt quite understand that part but she was talking about smth saying that there is a limit in the bone that can grow -the space in the TMJ area- and thats why we cant advance my lower jaw in an underbite position.

We did before and after pictures (I just moved my jaw forward in the after picture) and ppl that I kinda know or that are very interested in helping can pm me and I can send the pic, but I suppose the pic doesnt really help, it just shows that my jaw looks a lot better and not recessed in the after.

As I said, my main goal was getting more facial width and in the future reducing facial height via le fort or smth, rather than fixing my bad side profile and recessed mandible. Fixing my recessed mandible was a goal but I only thought about surgery in the future since im 17 and it wasnt a priority until I found out that there are non surgical ways. My question is "what do you think of this? Would that be stupid etc."

I suppose that a proper bimax or wathever -i dont know which surgery to get- would cost around that in total?

She even said that if I were to do MARPE and surgically expand my lower jaw, that only that would cost 20k!!

Functional mandibular appliance at 17?
You'd get 1-2mm pogonion advancement at best. most likely less. not even visible to the human eye. the majority of movement you'd get from wearing this device is dental.

It wouldn't change anything i just said, at 17 you're already past your peak pubertal growth. If you're expecting noticeable mandibular growth from wearing this device you got the wrong idea.

She does it also with patients that are mature. Its not about mandibular growth -you wont get any of it- its about moving the mandible in a different position.

That's actually the same thing i was describing. The growth source of the mandible is in the condyle. the positional change is only stable if the condyle grows around the new position. Without bone growth, the condyle just goes back when you remove the appliance. The stable portion of that positional change is exactly what i was referring to as skeletal change, and that's the 1-2mm i was talking about

and I wanted to ask is there a limit to the growth possible in the condyle? She talked about some limit prohibiting my jaw to even be moved more forward?

It would camouflage your skeletal class ii but the b point and pogonion would stay more or less the same position. The limiting factor is your CVM stage

he's saying you'd spend 7k and waste ur time for something that won't increase ur smv at all. its a waste.

the before and after the treatment, to resemble the after treatment i just have to put my mandible in the edge to edge bite position and thats it.
