Should I get a canthropexy

My eyes are slightly down turned and the only surgery I could find for that is a canthropexy if anyone knows any other surgery’s that would be a better alternative let me know

Looks closer to neutral/slightly positive.

Try curling the eyelashes up and outwards, it will make the eyes appear more positively tilted.

That looks neutral / very slightly positive.
Your eyelid hooding though is tilted negatively towards your lateral canthus, giving the illusion of an overall downwards tilted eye.

Surgery:

- a blepharoplasty will lower the negative hooding prominence.

- a canthopexy, as you mentionned, will also imrpove your eye area.

For softer alternatives:

- Pluck your eyebrows in a way that would make them look more straight/positively tilted
(For example the eyebrow tails, especially on your left eye (right on the image)

- Try curling your lashes up, especially in the lateral corner.

- You could also trim the eyelashes in the lateral area to lower the appearance of a negative tilt but be careful not to overdo it as it could look weird and people could notice.
