I don’t think any of the bottom ones are real movies then tbh.
Human centipede 2 was probably the most disgusting one I’ve seen

This iceberg is actually a smaller one, a youtuber by the name of Wendigoon made a thorough video on all of the movies in that iceberg and more.

It is an hour and 20 minutes, it´s pretty interesting if you have the time, although, I do warn to proceed with caution because the topics of the movies are pretty heavy and upsetting. If you do not want to spend the time, I´ll sum up most of those at the bottom the best I can.
All of those at the bottom 2 layers are compilations of gore, true crime scenes, and low budget films created for the sole reason of ¨it´s controversial,¨ it´s really nothing special and it´s just displeasing to look at. Movies on layer 3 and above are more professional style films but still controversial with pretty disturbing stuff in them.

the bottom tiers of these lists are always low budget, barely movies, garbage. its just like an hour of some middle aged guy puking and spraying cheap fake blood on craigslist actor MTBs

Best movie ive seen recently with a very dark subject matter (gore, sexual violence etc) was Ichi The Killer. Cool movie, but its not some "dude this is so shocking and fucked up" type movie, its actually pretty funny, lowkey a comedy.
