FFS can often include cheek implants as part of the procedure. You can browse results (as I often do) to see how this affects the FWHR as coming across implants to widen the face (which is sometimes seen as a feminine trait) is a bit rare...

Depends on your insurance. Most won't cover FFS, it's usually brushed off as only cosmetic surgery. Most FFS results come out of pocket.

It's paired with forehead and chin shaving, I don't know if the eyebrows were raised or if that was only by result of the shaving of the forehead. Personally I'm not a fan of the look either... as a rule of thumb it's much easier to add to a skull than it is to take away from it without delving into the uncanny valley. Nose jobs often suffer from this issue... still an impressive change of FWHR nonetheless.
