So in a nutshell this is the Blueprint for my surgery path
Current xRays,
So the main issues are
- SFS ( Hypodivergent )
- Upper maxilla recessed ( Class 3 )
- Not enpugh upper arch space to retract the front teeth on decomp
- Inward gonions
- Narrow midface (Not too much but still noticeable on smile)
Solutions
Will start with a MARPE, my lower arch is lingually compensated for my narrow upper arch rn so about 5-6MM of expansion would be easy, this would asess 2 of the problems above, Not enough space for my incisors and a mild narrowness of my midface, will add death tier nose widht but i will get an alarplasty and overall nosejob so thats covered.
The DJS will be in conjuction with a CW rotation to maximize facial height gain, considering my maxillas is severely short so any LeFort 1 cut could be considered a High cut on my case, so gotta be careful with my ANS, and the clockwise rotation I think is self explanatory, the genio that could make this a trimax is maybe if my chin ends up with an undesirable angle pointing too much down.
After this I would still lack mid-face projection and it would be more noticeable with the new position of my bones, so a mid-face implant similar to @midfacedeficient design wich would include malar projection and infra orb projection, paranasal filling etc. Gonion implants/filler ( still dont know exactly)
As my recessed maxilla causes eyebags a little fat graph would be quite nice here.
This is obviously a pretty shitty attempt to replicate the trimax but gives the idea of what I have in mind or at least a graphical representation it.
I would like to know any ideas, thoughts, recommendations you guys may have.
