Surgery qualifications

How do I know If I qualify for a bimax or any other kind of jaw operation so I don't have to pay it? (Germany)

Is your bite badly fucked up? Do you have bad sleep apnea? Do you have bad pain in your jaw? thats pretty much it and even then you might not qualify.
