After a phone call from the doctor's office, I realized that I had misunderstood what the doctor said during my appointment. I initially believed that my testosterone level was 6.9, which caused me some confusion and concern. However, that was not accurate. The doctor had actually said that my testosterone level was 629 ng/dL. That value is considered to be within the normal range, so there was no issue with my results after all!

Can you even live with 6.9?

- What it measures: The total amount of testosterone in your blood, including both bound (attached to proteins) and unbound (free) forms.

- Limitation: Doesn't show how much is actually usable by your body (muscles, brain, etc.).

### 2. **Free Testosterone**
- What it measures: The **unbound** testosterone that is free to act on tissues (the active form).
- Units: Usually **pg/mL** (picograms per milliliter) or sometimes ng/dL.
- Normal range for adult men: Roughly 50–210 pg/mL (or 5–21 ng/dL; varies by lab).
- Why it matters: This is the testosterone that actually does the work (builds muscle, affects mood/libido, etc.). You can have normal total T but low free T if too much is bound up.
- Common reason for low free T with normal total: High SHBG (see below).

### 3. **SHBG (Sex Hormone-Binding Globulin)**
- What it measures: A protein that binds to testosterone and keeps it inactive.
- Units: nmol/L.
- Normal range for adult men: Roughly 10–50 nmol/L (varies by lab).
- High SHBG → binds up more testosterone → lower free T even if total T is normal.
- Low SHBG → more free T (can sometimes cause other issues like acne or aggression).
- Common causes of high SHBG (low free T): chronic stress, depression, poor sleep, aging, low body fat, liver issues, some medications.

### 4. **Bioavailable Testosterone**
- What it measures: Free T + testosterone weakly bound to albumin (the portion that can become active).
- Units: ng/dL.
- Normal range: Roughly 110–575 ng/dL (varies by lab).
- This is sometimes checked instead of free T because it's easier and more reliable in some labs.

### 5. **Other related hormones sometimes checked**
- **Estradiol (E2)**: Estrogen. Men need a small amount for brain/mood/bone health. Too high or too low can cause emotional sensitivity, fog, or poor gains.
- **LH / FSH**: Pituitary hormones that tell the testes to make T. Low LH with low T = secondary hypogonadism (brain issue). Normal LH with normal T = good.
- **Prolactin**: High levels can suppress T and cause emotional issues.

### Why you might have normal total T (629 ng/dL) but still feel no workout gains + emotional sensitivity
- Most likely: **Low free testosterone** (high SHBG binding it up) from chronic stress, poor sleep, depression, or low body fat.
- Cortisol from stress/depression can also block T action at the cellular level even if levels are normal.
- Sleep debt severely impairs muscle recovery and anabolic signaling — even normal T won't build much if recovery is trash.
