I think most parents still view surgery as a very taboo subject and don't want to see their "little Jimmy" unnaturally change anything, even if he looks like complete trash. Also they're most likely viewing it from the point of view of what would benefit procreating instead of slaying (even if you get it you'll still be passing on your subhuman genes when the time comes) so it's a no go in that case
