The only way to fix downward growth for those who mouth breathed when they were younger/ had bad posture which led to them having a long face, is through modified Lefort 3 and Bimax CCW. Here is what both of these surgeries do:
Modified Lefort 3:
-The modified LeFort III involves osteotomies (bone cuts) around the orbits (eye sockets) and zygomatic arches (cheekbones).
-This allows for the entire midface to be mobilized and repositioned superiorly (upward), correcting the vertical elongation.
-By elevating the midface, this surgery reduces scleral show (the white part of the eye beneath the iris) and enhances under-eye support.
-This also improves the projection of the cheeks, addressing hollow or sunken midface appearance.
-Elevation of the midface can indirectly reduce the perceived length of the philtrum (the area between the nose and upper lip), contributing to a more balanced facial appearance.
Bimax CCW:
-Bimaxillary CCW rotation involves adjusting the occlusal plane (the angle of the bite) by rotating the upper and lower jaws.
-This rotation brings the chin and lower jaw upward and forward, improving jawline definition and reducing the vertical height of the lower face.
-The CCW rotation enhances the relationship between the upper and lower jaws, improving facial harmony and addressing any backward (retrusive) or downward growth patterns.
Combination:
-The combination of these surgeries effectively shortens the midface and lower face, resulting in a more proportional and aesthetically pleasing facial appearance.
-Together, they address the issues of downward growth by elevating the midface and correcting the vertical dimensions of the lower face.
