Accutane and Isotretinoin

This guide is meant for people who are currently taking isotretinoin or are considering starting it. In this guide im gonna explain u everything in one place:
  • how dosing actually works
  • what side effects to expect
  • how long the treatment should last
  • how to reduce side effects

1. What Accutane (Isotretinoin) actually is
Isotretinoin is the strongest acne medication currently available. It works through several mechanisms:

Reduces oil production
• Decreases acne-causing bacteria
• Normalizes skin cell turnover


Because of this, isotretinoin can permanently clear acne in around 80–90% of patients after a full course.

2. The most important concept: Cumulative dose
One of the biggest misunderstandings about isotretinoin is dosing. Many people think the treatment depends on daily dose.
But in reality, derms focus on total cumulative dose over the entire treatment.

Most dermatology guidelines recommend a cumulative dose of:
120–150 mg per kilogram of body weight
Example calculation:
If someone weighs 70 kg

70 × 120 = 8400 mg
70 × 150 = 10500 mg

So the total course should reach around 8400–10500 mg.
Stopping treatment before reaching this cumulative dose significantly increases the risk of acne relapse.
3. Typical isotretinoin dosing strategies

There are generally three dosing approaches used by dermatologists.

Low-dose Protocol
0.1 – 0.3 mg/kg/day

Pros:
• fewer side effects
• easier to tolerate

Cons:
• longer treatment duration (sometimes up to 12–18 months)

Standart Protocol
0.5 – 1 mg/kg/day

Example:
70 kg person: 35 – 70 mg per day

Typical treatment length: 4–6 months.

High-dose Protocol
Up to 2 mg/kg/day

Usually used for severe nodulocystic acne.
Higher doses may reduce relapse rates but usually increase side effects.

5. Example cumulative dose calculation
Example:
Someones weight = 70 kg

Target cumulative dose:
8400 – 10500 mg

If someone takes 40 mg per day
8400 ÷ 40 = 210 days
10500 ÷ 40 = 262 days

Estimated treatment duration:
7–9 months
(This is exactly the way for u or ur derm to calculate the cumulative dose)


6. What to expect month by month
Many people panic because they don't know what the treatment timeline looks like.
Here’s the typical progression:

Month 1Most people start experiencing very dry lips, dry skin, dry nose, and irritation. Many patients go through a purge phase, where acne temporarily gets worse. This happens because isotretinoin accelerates skin cell turnover and pushes clogged pores to the surface faster.

Month 2–3Oil production begins to drop significantly. Breakouts become less frequent, and skin becomes noticeably less greasy. Dryness usually becomes more noticeable during this period.

Month 4–5Many patients become almost completely clear at this stage. Remaining issues may include redness or post-acne marks rather than active acne.

7. Things you should always do on Accutane

Moisturize aggressively
Because ur skin barrier becomes very fragile. U MUST-HAVE products like:

U should definitely have lip balm or even just vaseline, otherwise ur lips will get so dry and start bleeding

Then we go to moisturizer. U can buy any other good moisturizers, but i would recommend u to buy these products

Next, it's crucial to buy a good SPF, as u simply can't go outside without it. Most importantly, don't forget to reapply it every 2-3 hours if u do go outside. Buy 50+ SPF. In my case its La Roche-Posay

Also u should have any topical tretinoins. This will help ur skin actively get rid of pimples that will appear for the first 2 months. (U CAN USE IT ONLY AT NIGHT)

Take isotretinoin with food
Isotretinoin is fat soluble. Taking it with food significantly improves absorption

In other words, without fat you may be missing out on approximately 50-60% of your intake

8. Basic Skincare Routine on Isotretinoin

Your skin becomes much more sensitive and fragile during treatment because isotretinoin dramatically reduces oil production and weakens the skin barrier.
if u keep using the same aggressive products u used before, u will likely end up with:

• irritation
• redness
• burning skin
• extreme dryness


Ur routine should stay very simple.

Morning routine:
1. Gentle cleanser (U also can wash ur face with water)
2. Moisturizer (then wait for 15min)
3. SPF

Night routine:
1. Gentle cleanser
2. Topical Tretinoin (be sure to wait 15-20 minutes for the tretinoin to be absorbed fully)
3. Moisturizer

That’s usually enough. Also dont forget about lip balm, u need to reapply it every hour.
Overcomplicating ur routine while on isotretinoin often makes things worse


9. Will acne come back?

Statistics suggest:
• around two thirds of patients remain clear long term
• around one third may need another course

So, after u complete the course, u and ur derm wait a month for clear skin. Basically, u're doing absolutely nothing for ur skin. it's supposed to stay clear forever, and then u'll go into remission. Don't forget that acne is a chronic condition that has no cure, and if even a course of systemic retinoids doesn't help, it's all down to ur genetics. So yeah, that’s basically everything u must learned about isotretinoin and what to expect during the course.
