  • 45° and true side profile (light from above/side to show shadows)

Look for: clear defined ridge edge? clean shadow under brow? low straight rim or high/rounded mess?

  • Forehead contouring + supraorbital osteotomy or onlay grafts :


    Forehead contouring is basically a surgery where they reshape the forehead bone so it looks smoother, flatter, and more balanced. If someone has really prominent brow bossing or a large frontal sinus, surgeons usually do something called a supraorbital osteotomy. That just means they remove part of the forehead bone, usually the front wall of the sinus, reshape it outside the body, and then place it back in a better position with tiny screws. The goal is to reduce how much the brow sticks out and create a cleaner transition between the forehead and the eye area.


    Onlay grafts are used when the surgeon wants to build an area up rather than shave it down. Instead of removing bone, they place a material on top of the existing bone to add more volume or definition. This material can be taken from the patient’s own body, made from bone cement, or created as a custom implant using a 3D CT scan. These grafts are often used to make the supraorbital rim look sharper, increase brow ridge projection, or fill in hollow or recessed spots so the upper face looks more balanced overall.

On r/PlasticSurgery guys talk about custom PEEK supra + infraorbital implants for better orbital framing and hunter eyes. Some got supralateral orbital rim implants through upper eyelid incisions with good results. Others use fillers or botox first to test the look before committing to surgery.


Dont over-sharpen or it can look uncanny. Bad placement can fuck nerves or cause migration. If you have big frontal sinuses, reduction might be needed instead of just adding. Always go to a surgeon who does male facial implants regularly.




Expected Results & Timeline

  • 2-6 months fillers/threads → cleaner ridge + better hooding
  • Surgery → permanent angular masculine upgrade

Biggest glow usually shows in 3/4 and side profiles where shadows hit right.


Always take before/after pics in identical overhead/side lighting. Compare in the same angles. The change compounds with good orbitals and low body fat.
