impacted lefort 1 + genio (advancement+widening) + CCWS (posterior+anterior) + bsso (optional but best to avoid malocclusion)
precise lip filler upper lip and flatten oral commisure
gonion filler, a few ml, or an implant but very expensive and impractical

you can only do them at 18 I assume
