but my medial canthus is already visible, if I perform an epicanthoplasty, wouldn't the tear duct be too visible?

The eye isn’t actually bulging. the maxilla is retruded further back than the eyeball. Picture on the right is an ACTUAL case of it

retard his brow and nasion are projected forward, which means his orbital bones aren’t recessed
doesn’t mean he doesn’t have excessive periorbital fat (which makes the eyes appear bulging from the front)
the picture you sent is also a case of orbital bone recession not just a fat issue

Interesting answer, so what defect would these implants eliminate? Would the filler or fat grafting discussed above make sense in my case?

Yes, fillers are probably possible, but there's only a forward projection while the width isn't affected. Because until now, I've always thought I had a very wide IPD and to mitigate this, I need width. What are your thoughts on this?

Yes, it's probably very unnatural on the cheekbones. I'll probably only do it in the area you mentioned... but it's also true that using fillers under the eyes in the malar area can cause some discomfort in the long run, from what I've heard... in any case, it seems to me that there's a notable improvement in the profile area by doing it, but in the frontal area, beyond the disappearance of dark circles, which can be easily solved with other procedures, I don't see any huge changes.
