Does anything exist that can be applied topically? Any high iqcels know if typical systemic igf1 blockers could be converted to creams for topical use to prevented/meaningfully reduce unwanted nose and ear growth while injecting GH or using GH increasing peptides?

No, IGF 1 receptor antibodies are mostly monoclonal iGg antibodies and they weigh 150 kDa on average which exceeds the limit of 550 daltons (to penetrate the stratum skin barrier) by alot which means it'd need other carriers or other methods to penetrate but then they would go systemic with those methods so probably not.

And even if there were, it could cause skin atropy.
