Hi everyone, I’m getting DJS soon. I consulted with a range of very good surgeons and they’re divided in two camps — and I’m REALLY confused. These are the 2 options:

1) surgeon 1 (became very famous in the last 5-6 years and has very few, if any, botches) YES to lower bicuspid/premolar extractions (just 2 lower ones) to create an overjet so that they can advance the mandible more, for a more aesthetically pleasing result He said about 11mm advancement of mandible.

2) surgeon 2 (is older and considered the God of jaw surgery worldwide and posts amazing results on instagram but has some major botches.) said NO premolar extractions of lower bicuspids. Instead, just do a little bit of “distillation” through elastics and IPR (interproximal reduction) to create a bit of space. But there will not be as much advancement. He said about 5mm advancement of mandible.

Both are VERY FAMOUS well known surgeons and respect each other but are firm in their different approach. Everything else in their surgical plans is similar: advancement, downgrafting, a bit of CCWR, genioplasty.

I’m so confused which one to go for to get the best result aesthetically and functionally.
