For some context, I have noticeable nasolabial folds despite being very lean, hollow/recessed under-eye areas, a narrow palate, difficulty resting my tongue on the roof of my mouth, and generally poor facial projection. Because of these features, I started suspecting that I might have deficient forward growth and a recessed maxilla.

After researching the topic for a couple of years, I finally consulted a maxillofacial surgeon.

Before I even explained my concerns, he told me that both my maxilla and mandible appeared recessed. He ordered a CBCT scan and a sleep study. After reviewing the results, he noted that my AHI was only 1, meaning I do not have clinically significant sleep apnea. He said surgery was therefore not strictly necessary from a functional standpoint, although he would be willing to advance my jaws by up to about 7 mm.

I then sought a second opinion from another highly regarded maxillofacial surgeon. After reviewing my scans and cephalometric measurements, he concluded that my maxilla is recessed and that my mandible is significantly recessed as well. According to him, orthognathic surgery would be beneficial.

I will post my CBCT images and cephalometric analyses below. Based on these findings, do you think I am a good candidate for bimaxillary advancement? More specifically, do the measurements suggest that surgery would produce a meaningful improvement in facial projection, or will it give me the forward growth of a dog and end up being praised by redditors ?
