can someone help me understand if blepharoplasty , fat grafting, and infraorbital rim implants all achieve the same thing which is to fix under eye area due to lack of fat or reccessed infraorbitals? Is it possible that I have projected infraorbitals but not enough fat in the area so it appears dark so then fat grafting would be best? If so, how do I know what procedure I need to fix the issue. Finally, what surgery is the best for this that has the least amount of long term issues and risk?
 
Blepharoplasty does something very different and typical cases that need fat grafting or especially implants (i.e. prominent eye cases) will usually be made even worse by doing a blepharoplasty.

Fat grafts and infraorbital rirm implants (and fillers) all build up the midface to correct a negative orbital vector. However if your cheek line is truly recessed behind the anterior-most point of your eyes then you will probably need implants for the ideal result, fat won't suffice.

Yes this is possible and the way to know is if your cheek is ahead of your eye but you still have a little hollow in the strip right under the lower eyelid (tear trough). If this is the case you need only fat grafts.

Finally, what surgery is the best for this that has the least amount of long term issues and risk?
Fat grafts, implants have all kinds of considerations like visibility or palpability, now or later with age, infection, extrusion later in life, etc. If you can get away with just fat grafts you should do that. But depending on your case they might not be enough. And if you get implants you may need to graft fat on top of them to cover them up anyway.

Fat grafting adds fat duh,
Bleth repositions the fat pad, smoothest the check/eye junction, tightens the muscle and in some cases if your really old they take skin.
Infras add kinda ridged volume it’s better if your trying to replicate bone ie not enough projection
or have dark under eyes. If it’s just hollow fat grafting best. If your sub 30 no way you need Bleth.

There’s a lot of long term problems with implants, and the surgery is complex with high complications risk. So really your left with fat grafting unless your a complete mogloid and have to have infras.
 
Blepharoplasty is that magical ritual where perfectly functional eyelids are hauled into a surgeon’s office and told they’ve “let themselves go.” A few millimetres of skin are declared an existential crisis, fat pads are accused of moral failure, and suddenly you’re under bright lights being promised a “refreshed” look—as if your eyelids were overdue for a software update. The marketing insists it’s subtle, undetectable, and totally natural, while somehow also guaranteeing you’ll look ten years younger, permanently well-rested, and vaguely superior at dinner parties. And if anyone asks? Oh no, you didn’t have surgery—you just “started sleeping better” and “cut back on salt.” Sure. Your eyelids definitely woke up one morning and chose transcendence all on their own.
