Pic Taken: ~2.5 m away from the mirror at around phone at eye level with face, head slightly tilted to the side; bad hairline skewed measurements; nostrils visible, therefore looking up a little too much)

Errors: My original measurements had four errors (wrong MFR distance, top of lip marked in two spots, jaw line too short, eye corners slightly off), which changed MFR from 1.21 to 1.06, jaw width from 71% to 75%, and canthal tilt from −3.6° to about −2°.
