I’ve been to 3 orthos so far, I still have 2 more consultations. 4 orthodontics and 1 airway focused.
These are the findings:
Class II, division 1, subdivision left malocclusion
• Missing upper left second premolar (tooth #13)
• Upper midline deviation to left
• Mild upper and lower crowding
• Open bite tendency
• Negative overjet/anterior crossbite
• Excessive incisor proclination
• Large tongue, forward tongue posture
All three so far have told me I’d need 3 extractions (which are the rest of them as I’m missing one premolar since I was a kid) of my second premolars to solve all of my problems. That is: alignment, midline discrepancy, incisor proclination, bite, etc. non-surgical route.
If I were to do ONLY braces: this would idealize alignment and correct underbite, while maintaining incisor proclination and midline discrepancy.
Now the “airway” focused ortho said that if I were to do this she anticipates I would not be happy with the results since I have a underbite. She thinks that I may not be able to close my lips and that my top lips would still be protruded. Now my first ortho didn’t mention this at all, but in the simulation it could potentially imply this. He does say that the extractions wouldn’t really “recess” me in terms of aesthetics. My large/tongue would be the advantage here and that my upper jaw wouldn’t retract much.
I don’t know what to do. I’m so lost and they won’t refer me to a surgeon because they don’t see it as a “need” or a “recommendation” , however they did mention I did have a skeletal AND dental issue.

TLDR:
- Get extractions: Higher possibility of facial profile changes if the treatment is overdone, but a much higher chance of correcting the flared teeth, improving lip closure, and achieving a proper, stable bite.
- Don’t get extractions: Avoid the risks associated with extractions, but there’s a higher chance my incisors remain a bit flared, lips don’t close comfortably, and bite (overjet/overbite relationship) not corrected.
