What can cause my philtrum to stick out?

I think the real problem is that my philtrum is sticking out of my face. The lenght is 17mm ( it’s still long but not a drama)
What can cause this?
And is there a surgery to treat it?

I think your philtral ridges/columns are too close (hence why your cupid's bow is narrow) and might be too thick, I don't know if these things can be fixed tbh, but maybe shortening your philtrum could balance it out.

Yes I agree with this assessment. But I don’t know of a great fix right now. Trying to amend it with fat grafts would probably not look right...
