what can I try for my skin???

Can someone please give me some advice on my skin situation.
The first pic was me about 1 year ago, my doctor ordered me to take ciscutan (accutane) and my skin got better and I stopped taking it, but it got worse again 2nd pic (right now), so I wanted to take the medication again but the doctor won't prescribe me the accutane cuz she said "it's not that bad" and it would have more negative then positive effect.

Do I try and get it from a different dermatologist, or should I try tretinoin, or this Korean skincare shit, I literally got no idea what to do, it looks way worse irl than in that picture.

I'm eating pretty clean too (cut) I red that whey protein is bad for ur skin, should I stop with that?
