Does it really exist or is it just the eyeball protrusion?

The orbicularis oculi muscle (the muscle around your eye) pushes upward when you smile. In some people, this muscle is more prominent or sits in a way that creates that soft “roll” under the eye, that’s why aegyo sal becomes more visible when you smile.
