"Thank you for referring this 23 year-old left-handed unemployed man, who told me that his
problems started in 2020 spontaneously. Mr Read became aware that the right arm felt tired and
there was some minor pain in the right forearm, which became tingly. This seemed to spread
proximally to the elbow and settled in the ulnar aspect of the right hand. Symptoms seem to
progress through 2021 and by 2022 the tingling had stopped. Mr Read still had numbness in an
ulnar distribution in the right hand and a numby patch in the right medial upper arm.
In early 2022, he developed pain in the right infraclavicular area with achiness at rest which was
worse with use. In February 2022, he saw Mr Indian at Hospital where he underwent
nerve conduction studies and subsequent right cubital tunnel decompression. That seems to have
made little difference to his symptoms and if anything, he feels he has been slightly worse with
more in the way of right-sided shoulder pain and shoulder blade discomfort. He feels that things
are getting slightly worse, and he now has difficulty abducting his thumb, as he explained to me.
There is some numbness in his right scapula. He feels that there is some slight tingling in his left
hand in an ulnar distribution. There is no pain on the left side. He has no lower limb symptoms,
and his mobility is unimpaired. He has had some occasional twitching in the muscles of the right
upper arm.
He does not smoke tobacco or drink alcohol. He is not on any medication and lives at home with
his parents.
Examination of the lower limbs was normal apart from symmetrically brisk reflexes. Cranial nerves
were normal. In the upper limbs, there was some minor thinning of the first dorsal interosseous on
the right and the thenar eminences bilaterally. I did not identify any weakness barring for some
minor weakness of the right thumb abductors. There was no objective sensory loss and reflexes
were intact and symmetrical and brisk. There was no lymphadenopathy."

They really loved hammering in the fact that I was unemployed and dependant on my parents.
