What do you put on the back of the covid 19 vaccine?

Seriously, i found a printable copy of the covid 19 vaccine card so I can trick my parents into thinking I took the vaccine. What do yall put on the back side of the vaccine card?

The back just tells you to return to a second dose in spanish and English and some information on the vaccine
