I would assume it’s either a narrow ipd, which throws off your esr, eye spacing, iaa, eme and such or either a narrow skull which throws off tfwhr, fwhr, midface ratio, chin to philtrum and just overall non existent angularity and dimorphism.

everything nigga if u learn to analyze ratios u will know they all affect erach other subtly or not

gonial angle can change 3 ratios frontally
wider cheekbones effects esr
upper third ratios

but to answer the question, it would be a mix so if ones tfwhr is completely fucked itl js screw their dimo alot,esr ofc if its REALLY bad enough,

theres no singular failo, theirs a reason why that failo is so severe and its caused by other ratios
