It does not look obvious. Of course it can be seen if you apply way too much and/or forget to make a gradient with your neck.
No point in combining with concealer imo. It would only make it look more obvious.
I use a primer now (the ordinary high-adherence silicone primer (£3.90)).
Results will be just as good if you don't apply a primer.
