What other surgery do I need to do?

I already had one botched DJS. I'm getting total joint replacement with Dr. Movahed in California for revision surgery. He's planning on advancing me 8mm and doing a PEEK implant on the right side of my face to fix the asymmetry between my left and right. He's also impacting my chin to help balance lower facial third, potentially with some shaving to shape the chin.

He can't do CCW because of my already flat occlusal plane. I asked to potentially do cheekbone (malar) implants to make up for mid face deficiency.

I also need a rhinoplasty eventually. Am I targeting the right things? Is there anything else I could be doing?

What happend in the first bimax? I mean the plan seems solid. Ofc you dont have high a potential but thats good to fix health etc since that goes first

It was condylar hyperplasia but they thought my joints stopped growing so they didn’t touch them. Was a mistake.

Health wise I think bimax will help because I have bad sleep apnea. But cosmetically is there anything else I should try to achieve through surgery?

What do you think is limiting my potential?

You have a lot of asymmetry… mostly dramaticized mainly by your uneven nasal alar projection..

The next … well I’m not a plastic surgeon but to me seems to be caused by un level bizgyomatic plane and bigonial… your left to right malar bones and gonions are basically uneven..

Malar projection both laterally(mostly posterior), and anterior is needed.

Your mandible other than lack of anterior projection is lacking is width… and shape.
You need mandibular implants to produce mandibular width(ie, height, width and angularity of the gonials).. as well as some work in the mandibular body to create balance. Lastly your protuberance will unlikely need to match the rest of your enhanced jaw structure as it is already rather wide and square/masculine.

BUT a chin implant for forward projection will look uncanny so avoid this! Your recessed lower facial third is mostly due to skeletal mandibular recession both forwardly and laterally, and is not of any means due to chin projection.

Pay close attention to the model below..

Lots of looskmaxxers focus on jaw width and forward projection.. but less on shape..


The gonial angle is sharp, producing angularity in the side profile and a sharp angle in the front… the protuberance also has high distinctiveness. Strong shape, square, it seperates from the rest of the mandibular body … u want this kind of angularity… and separation otherwise you’ll look round.. fat, and undymorphic


I might sound too autistic jfl lmk if u need further details but it seems your high iq from your knowing of what a flat occlusal plane is

Mandibular wise, I'm getting an implant on the right to match the left. Would you recommend a wraparound implant though?

Yeah I'm def not doing a chin implant, they're actually gonna vertically impact my chin to balance by lower facial third bc I have a long face.

Yeah absolutely agree with this. That's why I'm against sub malar implants too bc I think they give too much roundness and fullness and not the angularity I'm looking for.

Wrap around implant yes… most likely your surgeon will recommend this otherwise adding gonial width will look like this and stick out… no support from your mandibular body. you need the mandibular body to match the lateral projection of your gonion and look like the right picture

In terms of mid face implants I’m not too well versed… the most basic shit to my knowledge is bimax to extend the maxilla… which you need.. and from my understanding already got and failed at
