I don’t think I need bimax, I dont think my main issue is maxillary projection (or Maybe into the higher part of maxilla ? Since my under eyes support is not so great.)

I think my main issue is obviously gonions, even if i am lean it is impossible for me to achieve hollow cheeks or a great chiseled face. Always end up with a oval undefined face.

Obvious solution to my problem would be jaw implants, but I am not going into it since :
- results are often uncanny and looks off
- high risk of infection or rejection

Actually considering MARPE. Seems like the most intelligent solution for me to put structural change who seems great.

Open to any suggestions / ideas of what i can do.

marpe expands the maxilla, widening the dental arch and the midface floor. while this can broaden a narrow smile and slightly alter the midface structure, it does not directly change the shape, projection, or angle of the gonions

I mean, Doest widening the dental arch gonna give me a wider lower third ?

they most likely won't
though, if you lack bone projection at the jaw angle but want to avoid implants, firm dermal fillers (like calcium hydroxylapatite or dense hyaluronic acid) can be injected directly onto the bone at the gonial angle. this mimics bone structure, sharpens the jawline, and creates a clear boundary between your face and neck.

Oh didnt knew that.

I suppose it’s not permanent ?

most dermal fillers are not permanent and are gradually broken down and absorbed by the body over time.

You need le fort 2 ( which is not available but try your chance) cause your mid maxilla is flat.
Amd if you wanna widen your lower third is guess masseters training does help, or just get mandible implants.

I think he lacks too much volume/width/height in the posterior mandible to make Radiesse a viable solution. He's one of those cases where jaw angle implants are actually warranted.

@Tintin on top of jaw angle implants, I would also suggest :
-> squaring the chin off, either with fillers (contouring), or with a custom chin implant, or with a genioplasty (downgraft + widening)
-> a filtroplasty — philtrum shortening without any significant lifting of the upper lip

Rough simulation of the suggestions above :
Tin

(due to 2D limitations, I can't simulate a filtroplasty properly)

As for your side profile ... well ... it's over. Too many craniofacial deficiencies. Achieving a harmonious, well-projected, masculine side profile would require A LOT of work. I don't think it's worth it, and you might as well forfeit the side to focus on the front (more important in daily social interactions).
To illustrate my point :

The "after" would involve a trimax, jaw angle implants, filtroplasty, complete grafting of the nose (from the ANS to the radix), brow bone augmentation, probably supra/infra orbital implants, etc. ; the Lefort 2 suggestion made earlier in this thread wasn't ludicrous, although the results would probably be lackluster without complementary implants and grafts in the midface.
