Peep ceph. Ive done an ai ceph analysis too. Seems to think i have sfs. Interested to see if anyone disagrees.

What surgeries do u think i need to fix this and what surgeon would you recommend? I reside in australia (coceancig has been suspended).

Orthodontist thinks that i can do a bimax with downgrafting and wont need orthodontic work beforehand because my teeth are already aligned. But the ai analysis say my teeth are protruding. Orthodontist was very hesitant to work on me and thinks that I look fine and shouldnt risk anything.

Bonus: Orthodontist thinks my mandible is good but my maxilla is vertically deficient and hypoplastic.

When you smile, how much tooth show do you have?

That's the textbook sfs symptom.

Do you have the other data like gonial angle etc.?

About 50% of the incisors are hidden behind lips when i smile (top and bottom).

It's not how many teeth, but how much incisor show

Left is textbook sfs smile, he smiles like an old man.

Right is what you should more or less look if you have a normal smile. Men are supposed to show slightly less gum than women.

If your smile doesn't look like left, then you likely don't have sfs.

Remember if you do downgrafts while not being a sfs candidate, this is your fate

What's your gonial angle? I'm looking at your cephalo, and you seem like a CW rotation candidate.

Actually id say it’s closer to 75% hidden from top incisors.

Im not really sure what my gonial angle is. How can i find out? I think it’s within the normal range tbh. Around 130 or so.

Bro.

A 130 gonial angle is the opposite of SFS.

That's the gonial angle of someone that is badly downgrown.

If you have sfs, your gonial angle should be below 110.

Im guessing that’s why the ortho said my mandible is fine? Cant u measure the gonial from the ceph?

isnt 130 around the normal and ideal range? It’s 145+ that’s downgrown.

If your ortho thinks you have SFS then you probably have it.

Upper tooth show is the easiest diagnostic test.

If i have it i suppose i’ll need cw rotation? Will i need braces before surgery? Ortho thinks i dont. I want to avoid it if i can. My bite is good, teeth look straight and i dont have any tmj issues. I have a partially impacted wisdom tooth though; will i need that removed beforehand? Im really old now so waiting 2 years to ascend would suck.

Depends. You can get a net CCW downgraft (posterior + anterior downgraft) that gives you more tooth show

We’re talking about tooth angles here. Only an ortho can answer this.

If your tooth show is okay then you could just not do anything or opt for an upper lip lift instead.
