Varies person to person, this guy seems to have alot of bone mass back there, I need occiput implants also, since the back of my skull is flat and disproportionate which makes my haircuts look weird.

It's his forward head posture, he needs to chintuck and get his head back and you will see his normal cranium

Bimax + genio. As much movement as they can take him forward. That is going to do the most for him; if it doesn't look like enough, nothing else is likely to.

After DJS, he will likely still look kind of puny, so he should consider implants: Wraparound jaw implants (probably ideal), jaw angle implants to give him a more square look, even brow implants. We can't really see his cheekbones well, but they are probably weak too, so maybe cheekbone implants (to the side, not to the front) would help as well.

Just spitballing, this is a hard case. I think he could reach 5/10 or maybe even 5.5/10 with a lot of work.
