Pretty hard to say, can rule out double jaw surgeries cause it doesn't look like the upper jaw has moved. Maybe a sliding genioplasty, maybe mandibular osteotomy. Even chin implants can have that look with other surgeries in tandem.

I was completely off, maybe I overestimated the effects of djs on the maxilla or maybe I just can't tell the difference in the maxilla between the two

I think there is a noticable difference in the eye to tip of nose distance between the two which I would contribute to a movement of the maxilla, I don't know a lot about this though

I'd agree but she said she had a septoplasty so I don't know how much that might contribute to a slightly pointer nose rather than maxilla growth.

Double jaw surgery, bimax is a type of djs where both the upper and lower jaw are operated on at the same time, lefort 1 for upper jaw and BSSO surgery for lower jaw, I think some other surgeries may be involved with a bimax depending on what is needed
