its not a surgery specifically for canthal tilt tho
I know, but just in general if he is doing it ONLY for pfl, it doesn't feel worth it

Yo I’m hapa and I’m struggling with very thick negative hooding monolids with short pfl let me know

you will be very disappointed if you get a canthoplasty/canthopexy if the goal is to only increase pfl, the main way to actually increase pfl meaningfully is through lateral orbital rim expansion (mostly referred to as tripod surgery here) usually combined with medial wall implants (or bone grafts) in order to prevent enophthalmos (eyeball sinking inside the orbit) this combination (in theory) can also be used to increase ipd without an orbital box osteotomy.
