Yeah, of course, I didn't mention it in my previous post because, to my limited knowledge, no surgical procedure could improve his situation.literally any changes that would reduce your perceived IPD would be a massive improvement here
it is possible to decrease ipd with medial wall orbital decompression (source: https://pubmed.ncbi.nlm.nih.gov/28130596/) but in his case it’s effects will probably be limited since he doesn’t have proptosis (his eyeballs dont protrude/bulge out of the socket)Yeah, of course, I didn't mention it in my previous post because, to my limited knowledge, no surgical procedure could improve his situation.
Ideally, not only would he reduce the IPD, but he would also improve the eye shape by lifting up the upper lid near the medial canthus. Once again, I don't think any procedure can achieve that, unless a sort of partial ptosis repair surgery can alter one third of the eye aperture and not the other 2 thirds, while maintaining a natural look.
As suggested in my previous post, modifying the eyebrows' shape is the next best thing he can do for his eye area (that, and improved eyelashes).
Let's see if @Mr.Nct can offer a different insight, as he seems to have elite ball knowledge on complex oculoplastics.
Morph with corrected IPD and eye shape :

Unsurprisingly, that would generate a major improvement in facial harmony.
@waterboard in the morph above, I also accentuated your cheeks' hollows, which could be achieved with a liposuction of the cheek's fat pad. This would compliment your strong bone structure in the surrounding area (cheekbones, chin, mandible).
Thank you for the detailed reply.View attachment 5315718

Softmaxxes :
- clear your skin (isotretinoin ?), treat acne scars and refine your skin texture (tazarotene, chemical peeling, C02 laser)
- try to grow your medial eyebrows closer to each other (they should grow past the medial canthi) with minoxidil + microneedling
- trim the bottom part of your lateral eyebrows
- grow longer and denser eyelashes with Latisse/bimatoprost
- pay more attention to your overall grooming (hair cut, chest hair growing up your neck ...)
Hardmaxxes :
- rhinoplasty > rotate the tip downwards and graft the columella
- lip lift
- lateral chin reduction
- a little bit of filler in the mentolabial fold ; or a fat graft alternatively (if you ever do a fat graft for another part of your face, for example the periorbital area) ; not a priority, just a finishing touch
- otoplasty ; guessing from the side profile pic, as ears are concealed in the front profile pic. Besides the classic pinning, I would suggest to look into lobe reduction surgery as well. Your lobe seems to be hanging pretty bad, and it's only bound to worsen with age.
If you only care about your front profile, then keep the rhinoplasty + lip lift + otoplasty, and forget about the rest.
