he isn't really paying them. i'm not spoiling anything for you btw. but the film takes place in The Golden Glove, a café for lowlifes. the café is filled with people just drinking their emotions away, many women too, ugly fat sad alcoholic ones. protagonist fits right in with the bunch. he's extremely ugly with his cleft lip and no one is really interested in him outside the café. he pays these women with bottles of alcohol. they're drunk and desperate for attention so they come to his house pretty easily when he asks them to drink some more there. wouldn't say it's his fetish, he just can't get other meat
