Who requires new/MSDO? Can't jaw implants solve the issue if we've a good bite?
 
how many teeth show up when you smile? are your upper and lower teeth overcrwoded? do you lack zygos?

From on orthodontic view if ur upper/lower arch is too crowded you need expansion
Looks wise we need a picture

No crowded teeth. Only six are visible when I smile 🙄
