I was curious myself so I looked it up. I read in one abstract that a quadrangular lefort 1 is good for treating "midfacial zygomatic-maxillary deficiency." The below pictures show some of the variations on the procedure and some case outcomes. I don't know how to describe the aesthetic difference from standard lefort 1.

Another surgery you might want to compare the qlf1 to is the (intraoral) quadrangular lefort 2. I doubt it's easy to find a surgeon who will do a quadrangular lefort 2, but it looks like it would improve under eye support.

A normal lefort 1 will bring forwards only the upper jaw area. A quadrangular lefort will bring most of the midface forwards, preventing the "chimp" face look.
