i also just read this from a urologist about the side effects, not even the long term sides are about ED:

PDE-5is (this family of meds) work by dilating smooth muscle that surrounds the blood vessels. This allows more blood flow and helps with erections. It also dilates blood vessels all over the body, not just in the penis. This leads to the short term side effects of headache, upset stomach, facial flushing, muscle soreness and runny nose. These side effects go away depending on the half life of the med. Short for some, longer for others. See my other post about how long they act.

Long term there are very few worries with these meds. Some men could get changes in vision. These men should stop taking the med and it will reverse. Long term high dose use ( think Hugh Hefner) has been associated with hearing deterioration.

Ischemic Priapism is a prolong painful erection. This happends becuase the blood essentially slugges or clots inside the penis. This is a dangerous condition. It leads to increase in the pressure to the point where fresh blood cant get in and eventually tissue death. (Think tourniquet type action). These men need immediate care by a urologist (the faster the better). The good news is PDE5s dont really cause this to happen by themselves. Now in combo with other meds (cocaine, trazadone, seroquel) it can happen. I have never seen a case from PDE5i's alone, and we actually give then to men with recurrent priapism (from sickle cell diseases)
