..... It completely depends. Folks with very long lower thirds and steep mandibular angles will most likely benefit from CCW. Folks with more brachyfacial features, short lower thirds, will benefit more from CW rotation. Whatever will give a more "harmonious" after.

CW rotation elongates the lower third, whereas the CCW shortens the lower third on average. Generally speaking, CCW usually generates shorter lower third that is more forward, wheares CW does the exact opposite. To compensate for the lack of anterior prominence, CW should ideally be accompanied by more linear advancements that are potentially less stable, so it really depends on the anatomical variations of the patient, judgment and skills of the surgeons and some other variables.

Neither type of rotation is widening the jaw. But yes, you are generally correct in regards to what CW rotation does besides that. Picture the cut and rotations in your head, they don't widen the jaw. Picturing them can help you understand how the various movements and rotations will affect the appearance of the face.

I don't think your two successive surgeries is very realistic. I don't know much about IMDO, but I don't think it widens the jaw either. And from the brief reading I have done on it, an adult would not get IMDO and jaw surgery, just jaw surgery. CW rotation can also be combined with advancement to bring it forward. There is no reason to do CW rotation of either jaw, then do another surgery with CCW rotation of either jaw. A rotation is not a separate procedure. It's just part of jaw surgery... ie a lefort 1, BSSO, or their combination (alteration of both jaws).

Best thing is to just consult with several reputable surgeons, but I'm positive no one will suggest your hypothetical scenario.

Most nose lengths can look good if they have the right skull surrounding which maximises harmony. Only the extremely short or extremely long noses can guarantee you wont look good.
