Option A) €5000 --------> Holiday trip you always wanted and you know you will enjoy it (100% guaranteed to make you happy)

Option B) €5000 --------> Eye area surgery on the chance it might improve; if it does improve you are happy, if it doesn't, you are not happy (50% chance to make you happy)

You are guaranteed happiness with A, you take a 50% gamble with B.

Is your happiness you get from a successful eye surgery > happiness from trip to Japan?

If the level is the same, option A is your choice, cause it's a guarantee.

Yeah, that makes sense. It's a matter of short-term vs long-term happiness though. The effects of looksmaxxing might last a lifetime, while a holiday will create great memories (which I severely lack).
