Which surgeries to improve my side profile?

The mouth looks very weird in my opinion. The lips are downturned at the sides and the part below the bottom lip is too protruding (the lower lip normally should be sticking out a bit more. Also the philtrum is too short compared to chin height.

Are these problems substantial? I have no idea how large the issue is, people tell me its really bad, but then again some on looksmax tell me its not bad at all, so it is hard to gauge

How lean ? sometimes you may have bad bloat genetis (like me) , in which your face retains big amounts of water , actually looks like the case from the side

I mean if you just look at his head, you dont see the body, and then he still looks like a mogger. Compared to nowadays, where he cut his hair short. He looked a lot better and intimidating with that long hair.
