Who actually performs the surgery?

Some of these dudes are quite old. Surely their motor skills aren’t good enough for surgery? So the older ones direct an assistant or something to do the actual cutting?

Their motor skills don't degrade because they use them regularly and extensively. Age won't slow them down unless they get some sort of degenerative disease that causes your hands to shake or something.
