Who can install a bone anchored Hyrax and how can i convince my orthodontist to give it?

Who installs a bone anchored hyrax? Does it require general anesthesia? Is a maxillosurgeon needed or a dentist can do it? Or the ortho?

My orthodontist is preparing a tooth anchored Hyrax for me and says since im still young (21 btw lol) a surgery wont be needed and the suture will still crack from tooth anchored. What should i tell her to give me bone anchored
