Accutane a few times a week. Your mouth appears to be pretty narrow which may be caused by the narrow palate, if that’s the case get MARPE. Consider canthoplasty, to improve your CT. You ve got downward growth you need to improve it too
Your contrast is pretty bad, consider wearing coloured contacts. Get straight eye brows by grooming them correctly

My orthodontist tells me that i have wide palate intermolar width is 42mm I don't have buccal corridors its probably just lips are narrow

My orthodontist tells me that a lefort 1 osteotomy might be necessary in the future, but it's not set in stone yet. I have a Class III malocclusion (my jaw is shifted by one tooth, and I have an edge-to-edge bite). However, because my upper jaw is very wide, it’s most likely a functional Class III malocclusion since I can freely move my lower jaw back and forth.He mentioned that my lower jaw probably shifted too far forward due to heavy chewing during childhood. Since the age of 10 or 11, my stepfather forced me to eat cartilage, which was very difficult at first. Plus, I lived in a village back then, so I ate a lot of apples, nuts, and hard foods. Since I have Armenian roots on my father's side, this heavy chewing load likely triggered a genetic predisposition to a Class III bite.He also noted that I don't actually have a strong downward growth pattern ln fact, he said it's almost perfect. It’s just that my face is genetically flat in certain areas, and since the lower jaw is still growing, it only creates the illusion of downward growth.I also told him that my jaw function is completely fine. I breathe through my nose 24/7 without any issues. Even during sprints and cross-country running, I can run at a moderate pace for half an hour breathing exclusively through my nose, and I recover my breath the same way. I don't have sleep apnea. I’m not sure, maybe I just look puffy right now or I'm still filling out, but the orthodontist said my lower jaw is simply still growing, so we just need to wait and see.
