The Italian trio: Raffaini, Ramieri, and Pagnoni. There are plenty of other good surgeons out there for aesthetics, but these three have some of the best reputations.

That is a very broad question. There exist different experts for numerous realms of plastic surgery. A surgeon can be an expert in bimaxillary osteonomies, but awful at anything related to soft tissues.
Different surgeons have different biases and will often try to sell you on solutions tailored to their own speciality. This isn't necessarily malicious; it's just that they're hammers, so they tend to see nails.
