Why aren't obese women grateful that I insult them?

I work w/ some of the fattest/ugliest "people" on planet earth.

Some time ago (like 3 months maybe) I pointed to a morbidly obese landwhale in her early 20s (she's young , so she's vulnerable lol ) plate, which had a 2 doughnuts and a hot dog and said "wow, seems like you never had breakfast today" she said "yep" (almost about to cry it seemed) and then I said "I can tell" and laughed.

Another time I pointed to an obese woman's McDonalds breakfast and said "looks healthy" sarcastically

I am pretty much saving their lives by telling them to eat healthy. Why are they not grateful?
